OUR PROCESS
Our goal is to provide the most efficient, accurate, and affordable I-693 Immigration Medical Exam in the area. Completion of the process involves two visits. Visit 1 can be either in-person, over the phone or video, or even asynchronously online. Choose whichever option works best for you.
Step-by-Step Guide to
I-693 Immigration Physicals
Gather demographic information (such as name, address, date of birth, etc).
Review your medical history, and decide what (if any) records are needed.
Discuss and review the USCIS vaccine requirements, which vary by age.
Review and order the relevant required lab tests (QFT, RPR, GC).
Perform the required focused physical examination.
Review and document lab results.
Complete and sign the I-693, then seal it in an envelope for delivery to USCIS.
Possible forms of identification include your birth certificate (with an English translation) or an affidavit.
Bring any vaccine records you have. Records that are not in English need to be accompanied by a certified translation.
Health insurance does not cover the cost of Immigration Exams. However, it will usually cover the cost of any required vaccines.

Is Evergreen experienced in doing these exams?
Yes, our Civil Surgeon has completed more than 1,000 immigration medical exams over the last 3 years!
No it does not, so please choose whichever visit type you are most comfortable with. However, some customers prefer the phone/email option because it allows us to initiate the process that very same day.
No, USCIS requires that the Civil Surgeon order these tests on your behalf.
Will I need a chest X-ray?
Less than 15% of patients require a chest X-ray. The need for an X-ray is based on the blood test results, and costs an additional $50.
Vaccine requirements vary by age of the applicant and month of the year. USCIS guidelines for vaccines change frequently, and we will discuss the most current requirements at visit #1. Once a decision is made, we suggest going to your PCP, or making an appointment at a local pharmacy.
We strongly recommend getting the required vaccines, as USCIS will reject your application if the vaccines are incomplete. However, Evergreen can provide a waiver for Varicella and MMR if a woman is pregnant or may become pregnant in the next 3 months.
About 1 week total, sometimes less! We will order your lab tests at Visit 1, and these can be completed at any Quest Diagnostics center, including the one right by our office. We can complete Part 2 as soon as Quest delivers your results.
Unfortunately no, the USCIS process is not covered by health insurance. However, your private health insurance often covers the cost of vaccines required for the process.
